Qualifications

• Active, unrestricted RN licensure in the state of practice.
• BLS or CPR certification required.
• Fluent written and spoken English.
• US work authorization.
• Experience with Electronic Medical Records (EMR).
• Ability to maintain patient confidentiality and handle information confidentially.
• Ability to assess patients without face-to-face interaction with strong communication and assessment skills.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage to ensure appropriate care and avoid unnecessary visits.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccination administration.
• Educate patients and caregivers on chronic conditions and preventative care in person, by phone, or in group settings.
• Review medications within nurse-led visits for safety and adherence.
• Conduct onsite testing (POC labs, EKGs, specimen collection) and use data to close gaps in preventative screenings.
• Review high-priority lab results to maintain safety and continuity of care.
• Collaborate with providers and care teams to manage complex care plans and transitions from hospital to clinic.
• Use standardized protocols for medication management, renewals, and prior authorizations.
• Ensure clinical communications are managed, tasks completed, and visits closed timely.
• Delegate to medical assistants and phlebotomists to support patient flow.
