Oak Street Health in Rock Hill, SC seeks a Registered Nurse to join our Center Nursing team in a value-based care model. You’ll perform assessments, manage chronic conditions, and collaborate with the care team to prevent hospitalizations.

In this full-time role, you will administer vaccines, wound care, and medication management, while educating patients and caregivers. You’ll help coordinate transitions from hospital to clinic and support a welcoming clinic environment.

Qualifications

• Active, Unrestricted RN Licensure in the state you reside.
• BLS or CPR Certification.
• Fluent written and spoken English language skills.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality.
• Ability to assess patients without face-to-face interaction with strong communication and assessment skills.
• 2+ years of healthcare experience as an RN (preferred).

Responsabilités

• Support patient assessment and triage to ensure appropriate care and avoid unnecessary hospital visits.
• Execute protocol-driven chronic condition management.
• Perform wound care and medication administration, including vaccines.
• Educate patients and caregivers on chronic conditions and preventive care.
• Review medications within nurse-led visits for safety and adherence.
• Conduct onsite testing (POC labs, EKGs) and analyze results to close gaps in screening.
• Coordinate weekly transitions for patients moving from hospital to clinic with the team.
• Manage prescription renewals and prior authorizations following protocols.
• Ensure timely clinical communications and close patient visit encounters.
• Delegate to medical assistants and phlebotomists as needed.
