Job summary

Oak Street Health in the United States is seeking a Registered Nurse to support care delivery within a value-based model. You will perform assessments, manage chronic conditions, and collaborate with the care team to prevent avoidable hospital visits.

You will perform wound care, vaccines, education, and coordinate with providers to ensure high-quality patient outcomes while maintaining safety and confidentiality in a fast-paced clinic setting.

Qualifications

• Active, Unrestricted RN licensure in the state you reside.
• BLS or CPR certification.
• Fluent written and spoken English language skills.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality.
• Ability to assess patients without face-to-face interaction with strong communication and assessment skills.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage to ensure appropriate care.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccination administration.
• Provide patient and caregiver education on chronic conditions and preventative care.
• Review medications within nurse-led visits to ensure safety and adherence.
• Conduct onsite testing (POC labs, EKGs) and data-driven care gaps closure.
• Collaborate with providers to manage complex care and weekly patient transitions.
• Utilize protocols for medication management, renewals and prior authorizations.
• Ensure clinical communications are managed and encounters closed timely.
• Delegate to medical assistants and phlebotomists as needed.
