The Center Nurse supports triage, onsite testing, and coordination of care across settings, delivering high-quality, compassionate care in a fast-paced environment.

Qualifications

• Active, Unrestricted Registered Nurse Licensure in the state that you reside.
• BLS or CPR Certification.
• Fluent written and spoken English language skills.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and process information confidentially.
• Ability to assess patients without face-to-face interaction, strong communication and assessment skills.

Responsibilities

• Support patient assessment and prioritization for walk-in and telephonic triage to ensure appropriate care.
• Execute protocol-driven chronic condition management.
• Perform skilled nursing procedures including wound care and vaccines.
• Provide patient and caregiver education on chronic conditions and preventative care.
• Conduct medication reviews to ensure safety and adherence.
• Onsite testing (point-of-care labs, EKGs, specimen collection) and using data to close gaps in preventative screenings.
• Review and act on high-priority abnormal lab results to ensure safety.
• Collaborate with providers to manage complex care plans and weekly transitions from hospital to clinic.
• Use standardized protocols for medication management and renewals.
• Ensure timely clinical communications and closing of patient visit encounters.
• Delegate to medical assistants and phlebotomists to support patient flow.
