HopeWest in Grand Junction, CO is seeking a Clinic Registered Nurse (RN) for the PACE program. The RN provides evidenced-based nursing services, conducts assessments, develops care plans, and educates participants and families, coordinating care to meet regulatory requirements.

This position offers a $10,000 sign-on bonus and a comprehensive benefits package, including health insurance, PTO, 403(b) match, life insurance, tuition reimbursement, and EAP.

Formación

• Registered Nurse with current Colorado licensure.

Clinic Registered Nurse (RN) - $10,000 Sign-On Bonus

The Clinic Registered Nurse is responsible for providing evidenced based nursing services to participants enrolled in the PACE program. The Clinic RN provides support to the participant and their caregiver/family members, assists the participant to maintain or increase their health status through the provision of Clinic services including teaching, counseling, and other skilled interventions. This position is responsible for providing superior physical assessment techniques, the development of effective nursing care plans and professional nursing care of the patient. The Clinic Registered Nurse must have well developed organizational and interpersonal skills to effectively coordinate the scheduling, communication and encounter documentation of all care provided in compliance with regulatory requirements

What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.

The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

What You Bring:

• Registered Nurse with current Colorado licensure, maintained throughout employment.
• Graduated from an accredited school of nursing; BSN preferred.
• Minimum of 2 years' experience as an RN, clinic experience preferred.
• Excellent interpersonal and communication skills.
• Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.
• Effectively work with and build relationships with others.
• Interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
• Successfully completes competency evaluations.
• Excellent computer and technology skills.
• Effectively manages time and able to maintain focus on multiple priority tasks.

• Competitive pay structure ASN:$35.02 - 38.75 OR BSN:$36.20 - $40.05 per hour depending on experience.
