Hartford HealthCare Medical Group is seeking a Registered Nurse to provide skilled office-based nursing care under provider direction across multiple Connecticut locations. You will assess patient needs, develop and evaluate care plans, and perform independent nursing actions while triaging calls with empathy and privacy in mind.

The role requires an active Connecticut RN license and CPR certification, with preference for 3 years of medical practice experience and familiarity with medical

Qualifications

• Current state of Connecticut RN license and CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and formulates nursing plan of care with continuity of patient care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing interventions promptly and professionally.
• Assists patients and families with concern and empathy; respects confidentiality and privacy.
• Travel to other regional offices, as needed.
