Candidates should hold CT RN license with CPR; 3 years in medical practice is preferred, and familiarity with medical software is expected. Travel to regional offices may be required; a supportive environment and growth opportunities are

Qualifications

• Licensed RN in Connecticut with CPR certification.
• Minimum 3 years of medical practice experience preferred.
• Experience using medical software/EMR systems.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and plans care; ensures continuity through evaluation and intervention.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing intervention.
• Assists patients and families with empathy; respects confidentiality and privacy.
• Travel to other regional offices, as needed.
