Job summary

Hartford HealthCare Medical Group is seeking an experienced Registered Nurse to join our team in Connecticut. Under provider direction, you will deliver office-based nursing care, assess patient needs, develop plans, and coordinate follow-up with providers across multiple sites.

You will triage calls, support families, and travel to regional offices as needed. A CT RN license, CPR certification, and at least three years of practice experience are preferred; we offer tuition reimbursement,

Qualifications

• Current CT RN license and CPR certification are required.
• Three years of experience in a medical practice preferred.
• Experience with medical software is preferred.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and formulates nursing plan of care.
• Triages telephone calls and initiates appropriate nursing interventions.
• Assists patients and families with care coordination and privacy.
• Travel to regional offices as needed.
