Qualifications

• Current state of Connecticut RN license and CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software systems.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and families.
• Demonstrates independent nursing actions based on assessment and care planning.
• Triages telephone calls and initiates appropriate nursing interventions.
• Assists patients and families with concerns while maintaining confidentiality.
• Travel to other regional offices, as needed.
