Job summary

Hartford HealthCare Medical Group in Connecticut is seeking a registered nurse to provide skilled nursing care in diverse clinical settings under provider direction. The RN will support patient care planning, triage, and collaboration with a multidisciplinary team across Hartford HealthCare Medical Group’s network.

The position requires a Connecticut RN license and CPR certification, with travel to regional offices as needed.

Qualifications

• Current state of Connecticut RN license and CPR certification.
• Three years' experience in a medical practice preferred.
• Experience with medical software.

Responsibilities

• Provides office-based nursing care in collaboration with provider
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care
• Demonstrates independent nursing actions based on assessment and problem identification
• Triages telephone calls and initiates appropriate nursing intervention
• Assists patients and family members with concern and empathy; respects confidentiality and privacy
