You will coordinate appointments, authorizations, and pharmacy activities, monitor progress, and collaborate with cross-functional teams to keep treatment plans on track. Strong communication and organizational skills are essential in this fast-paced role.

Qualifications

• 3–5 years in clinical case management or care coordination.
• Experience with EHR systems and MS Office.
• Strong communication, organizational, and patient service skills.

Responsibilities

• Guide patients through their treatment journey.
• Coordinate appointments, provider schedules, authorizations, and specialty pharmacy activities.
• Monitor patient progress and address barriers to care.
• Maintain documentation, financial estimates, and patient records in CRM/EHR.
• Respond to patient inquiries with empathy and urgency.
• Support high-quality patient outcomes while ensuring compliance.
