Job summary

Greenbrook Mental Wellness Centers, a subsidiary of Neuronetics, seeks a compassionate Case Manager to guide patients through their treatment journey and coordinate appointments, authorizations, and care plans. You will work within the Patient Access – Care Coordination team to ensure timely, high-quality outcomes.

This clinic-based role requires 3–5 years in clinical case management, strong communication, and proficiency with EHR and CRM systems.

Qualifications

• High School Diploma or equivalent with 3–5 years in clinical case management or care coordination.
• Experience using EHR systems and Microsoft Office applications (Outlook, Word, Excel).
• Strong communication, organizational, and patient service skills with multi-tasking ability.

Responsibilities

• Guide patients through their treatment journey with a seamless, supportive experience from consultation to completion.
• Coordinate appointments, provider schedules, authorizations, specialty pharmacy activities, and treatment readiness requirements.
• Monitor patient progress, address barriers to care, and collaborate with cross-functional teams to keep treatment plans on track.
