Job summary

Greenbrook Mental Wellness Centers in Columbia, MD, seeks a compassionate Clinical Treatment Case Manager to guide patients through their treatment journey from intake to completion. This clinic-based role coordinates appointments, authorizations, and care plans as part of a collaborative patient-access team.

You’ll manage scheduling, navigate insurance processes, maintain accurate records in CRM/EHR, and support patient advocacy with empathy and urgency in a fast-paced healthcare environment.

Qualifications

• High School Diploma or equivalent with 3–5 years of clinical case management or care coordination experience.
• Experience using EHR systems and Microsoft Office (Outlook, Word, Excel).
• Strong communication and organizational skills with excellent patient service.

Responsibilities

• Guide patients through their treatment journey from consultation to completion.
• Coordinate appointments, provider schedules, authorizations, and specialty activities.
• Monitor progress, address barriers to care, and collaborate with teams to keep plans on track.
• Maintain documentation, financial estimates, and patient records in CRM and EHR.
• Respond to patient inquiries with empathy, professionalism and urgency.
• Support high-quality outcomes while ensuring compliance with requirements.
