Job summary

Greenbrook Mental Wellness Centers, based in Delray Beach, Florida, is seeking a dedicated Case Manager to be the primary contact for patients, coordinating care from consultation through treatment completion. You will manage appointments, authorizations, and documentation in CRM/EHR systems while addressing barriers to care.

The role emphasizes patient advocacy, collaboration with cross-functional teams, and high-quality service.

Qualifications

• High School Diploma or equivalent required combined with 3-5 years of experience in clinical case management, patient care coordination, direct patient care, or a related healthcare setting
• Experience using EHR systems and Microsoft Office applications (Outlook, Word, and Excel)
• Strong communication, organizational, and patient service skills with the ability to manage multiple priorities

Responsibilities

• Guide patients through their treatment journey, providing a seamless and supportive experience from consultation to completion
• Coordinate appointments, provider schedules, authorizations, specialty pharmacy activities, and treatment readiness requirements
• Monitor patient progress, address barriers to care, and collaborate with cross-functional teams to keep treatment plans on track
• Maintain accurate documentation, financial estimates, and patient records within CRM and EHR systems
• Respond to patient inquiries with empathy, professionalism, and urgency while delivering exceptional service
• Support high-quality patient outcomes while ensuring compliance with operational, coverage, and treatment requirements
