Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, or related field.
• Minimum of 2 years experience in social assistance, case management, or related role.
• Strong knowledge of social service programs, resources, and eligibility criteria.
• Excellent communication and interpersonal skills to engage diverse client populations.
• Proficiency in maintaining confidential client records and using case management software.

Responsibilities

• Conduct comprehensive assessments of client needs and eligibility for social assistance programs.
• Develop and implement individualized service plans with clients and relevant agencies.
• Coordinate and monitor delivery of services to ensure timely support.
• Maintain accurate case records, documentation, and reports in compliance with standards.
• Advocate for clients to address barriers and facilitate access to resources.
• Collaborate with multidisciplinary teams including healthcare providers and community organizations.
• Provide ongoing support and follow-up to evaluate progress and adjust plans.
• Participate in community outreach and education initiatives.
