The Family Center in Brooklyn seeks a Client Care Coordinator to provide comprehensive case management for homeless families in a Tier II residence. The role involves intake, risk assessment, initial biopsychosocial evaluations, and coordinating living plans with clients and case managers.

Responsibilities include facilitating services, advocating for clients, and maintaining confidential records while ensuring timely documentation and reporting. Salary ranges with benefits are offered.

Competenze

• LMSW license must be obtained within one year of hire.
• Sensitivity to working with homeless and vulnerable populations preferred.
• Bilingual abilities are a plus; pre-employment screenings required.

Mansioni

• Maintain client relationships and confidentiality.
• Assist clients with intake forms, screenings and documentation.
• Conduct risk assessments and develop initial biopsychosocial evaluations.
• Coordinate ILP development with clients and case managers.
• Refer clients to medical, psychiatric and social services as needed.
• Lead group counseling sessions and provide psychoeducation.
• Document progress notes within 48 hours and assist with reports.
