Responsibilities

• Coordinate physical health, mental health and social needs for individuals with complex conditions.
• Ensure timely access to medical and psychosocial services and maintain care plans.
• Engage clients and families to develop an integrated person-centered plan.
• Monitor data, track follow-ups, and document in electronic records.
• Provide crisis intervention, time management, and referral across providers.
