Responsibilities

• Provide high quality primary and palliative care to patients in home/home-based settings.
• Perform physical examinations, update medical histories, and develop care plans.
• Communicate effectively with office staff and clinical operations across platforms.
• Care for a diverse patient panel with chronic conditions.
• Attend quarterly medical staff meetings and collaborate with physicians.
