Job summary

Ennoble Care is a mobile primary care, palliative care, and hospice provider delivering care at patients’ homes in Florida. We seek a Full-time Nurse Practitioner or Physician Assistant to deliver comprehensive primary care in the Polk County region, under physician supervision, caring for patients with chronic conditions.

Qualifications

• Board Certified NP or PA with valid Florida certification.
• 1–2 years of primary care experience with geriatric patients required; hospice experience preferred.
• Strong clinical skills with the ability to assess, diagnose and develop care plans.

Responsibilities

• Provide high quality primary and palliative care to patients in Polk County, FL.
• Perform physical examinations and develop care plans based on histories and tests.
• Communicate effectively with office staff and clinical operations across platforms.
• Deliver patient-centered care to a defined panel of patients.
• Attend quarterly medical staff meetings.
• Develop impressions, diagnoses and treatment plans.
