Qualifications

Responsibilities

• Diagnose and manage chronic and acute conditions holistically.
• Coordinate care with nurses and clinical teams across settings.
• Maintain timely documentation and collaborate on care protocols.
• Educate patients and families on disease prevention and wellness strategies.
