Job summary

Ennoble Care in Cincinnati, OH is seeking a Nurse Practitioner or Physician Assistant to deliver primary and palliative home-based care under physician supervision. You will assess patients, develop care plans, order tests, and collaborate with our in-house clinical team to provide high-quality, patient-centered care.

Qualifications

• Board certified NP or PA in Ohio is required.
• 1–2 years of primary care experience with geriatric patients preferred.
• Ability to perform physical examinations and create care plans.
• Ability to work under physician supervision and autonomously when needed.

Responsibilities

• Provide primary and palliative home-based care in Cincinnati, OH.
• Assess patients, obtain histories, and develop best care plans.
• Order diagnostic tests as medically necessary.
• Prescribe medications and manage patient care with the clinical team.
• Attend quarterly medical staff meetings and collaborate across platforms.
