Job summary

Garden Springs Healthcare seeks a Regional Hospital Liaison for the Greater Cleveland market to drive referrals and census growth across our network of facilities. You will educate hospital partners on capabilities and coordinate admissions with care teams.

The role requires established Cleveland-area relationships, strong communication, and the ability to travel within the region to support timely discharge and placement decisions.

Qualifications

• Experience in skilled nursing, post-acute care, hospital business development, healthcare sales, admissions, case management, or related healthcare role.
• Strong knowledge of the Greater Cleveland healthcare market and established hospital relationships preferred.
• Proven ability to develop referral relationships and drive measurable growth.
• Understanding of post-acute referral and discharge-planning process.
• Ability to communicate clinical capabilities and placement solutions to healthcare professionals.
• Highly organized, responsive, and able to manage multiple hospital relationships.
• Strong communication and problem-solving skills.
• Self-motivated and confident working independently in the field.
• Ability to travel throughout the Greater Cleveland market.

Responsibilities

• Develop and maintain strong relationships with hospital case managers, social workers, discharge planners, physicians, utilization review teams, and other key referral sources throughout Greater Cleveland.
• Represent the full Garden Springs Healthcare portfolio and educate referral partners on clinical capabilities, programs, coverage, and payer options.
• Identify new hospital relationships and opportunities to expand Garden Springs' presence within existing accounts.
• Maintain a consistent presence within assigned hospitals and become a trusted resource for complex discharge planning and post-acute placement.
• Drive referral volume, admission conversion, and census growth across the Garden Springs network.
• Serve as a resource for challenging placements and help referral partners identify solutions within Garden Springs.
• Collaborate with Centralized Admissions & Intake to ensure referrals receive timely review and follow-up.
• Coordinate admissions with facility teams and maintain communication with hospital partners during transitions of care.
• Conduct bedside visits and patient/family meetings when needed to support placement and admissions.
• Track referral patterns, lost opportunities, competitive activity, and growth opportunities.
• Participate in hospital meetings, networking events, and community activities.
• Maintain follow-up after admission to strengthen relationships and future referrals.
• Partner with leadership to develop market-specific strategies for census and specialty-program growth.
