Job summary

Garden Springs Healthcare seeks a Hospital Liaison to support growth across Newark and the Greater Columbus market. You will represent the Garden Springs portfolio to hospitals, health systems, and referral partners, guiding placement decisions based on patient needs.

Ideal candidates will have strong relationship-building skills, know the Central Ohio market, and be comfortable traveling to partner sites to drive census growth and ensure smooth admissions.

Qualifications

• Experience in skilled nursing or post-acute care.
• Established relationships with Newark and Greater Columbus hospitals preferred.
• Ability to travel across Central Ohio.
• Strong communication and relationship-building skills.

Responsibilities

• Develop and maintain relationships with hospital case managers, social workers, discharge planners, physicians, utilization review teams, and other referral sources.
• Represent the full Garden Springs Healthcare portfolio and educate referral partners on our capabilities, programs, geographic coverage, and payer options.
• Maintain a strong and consistent presence within key hospital accounts throughout the market.
• Identify new hospital relationships and opportunities to expand Garden Springs' presence within existing accounts.
• Become a trusted resource for complex discharge planning and post-acute placement needs.
• Drive referral volume, admission conversion, and census growth across the Garden Springs network.
• Collaborate with Centralized Admissions & Intake to ensure referrals receive timely review and follow-up.
• Coordinate admissions with facility teams and maintain communication with hospital partners during transitions of care.
• Conduct bedside visits and patient/family meetings when needed to support placement and admissions.
• Track referral patterns and opportunities for growth within assigned accounts.
• Participate in hospital meetings, networking events, and community activities.
