Qualifications

• Bachelor's degree or associate degree with 3 years’ experience.
• Licensed as a Registered Nurse.

Responsibilities

• Assess, plan, implement, coordinate, monitor, and evaluate patient care needs.
• Identify high-risk patients and advocate for appropriate care.
• Maintain high-risk patient registry and ensure timely documentation for reimbursement.
• Develop comprehensive case management plans to improve outcomes and self-care.
• Coordinate care across services and facilitate transitions between facilities.
• Lead multidisciplinary patient care conferences for high-risk or complex cases.
