Job summary

LaSante Health Center is seeking a Chronic Care Management Coordinator to guide patients and families through their care journey and connect them with internal and external resources. The role emphasizes patient-centered care, compassion, and collaboration with healthcare teams.

The coordinator will support patients with chronic conditions, enroll them in relevant programs, and facilitate communication among patients, families, and providers to ensure coordinated, high-quality care.

Qualifications

Responsibilities

• Act as the main point of contact for patients and families.
• Monitor and manage a designated panel of patients with chronic conditions.
• Support patients in understanding and managing their conditions, medications, and treatment plans.
• Enroll patients in medical and support programs as needed.
• Facilitate communication between patients, families, and providers for coordinated care.
• Assist patients with scheduling and navigating appointments, referrals, and follow-up.
• Connect patients to community resources and social support.
• Perform additional duties as assigned by supervisors.
