Job summary

Living Water Clinic – Clinica Agua Viva is seeking a Chronic Care Patient Navigator to engage patients, explain enrollment in chronic care services, and coordinate visits with care teams. You will manage scheduling, device education, and follow-up communications, ensuring patient concerns are addressed with sensitivity and accuracy.

The role requires strong organizational and communication skills, experience with scheduling and EHRs, and the ability to work collaboratively across multiple teams

Qualifications

• High school diploma or equivalent required.
• Experience in patient scheduling, medical office support, navigation, or care coordination preferred.
• Medical assistant or related training preferred.
• Strong communication, organization, and customer service skills.
• Familiar with electronic health records and remote monitoring technology.

Responsibilities

• Outreach to patients to explain and enroll in chronic care services.
• Schedule visits, RPM reviews, and device education sessions.
• Coordinate with providers and clinic teams for appointments.
• Document patient contacts and follow-up actions in designated systems.
• Escalate clinical concerns to licensed staff per procedures.
• Maintain device inventory and assist with device setup.
• Provide coverage across LWC locations as assigned.
