Job summary

Sea Mar Community Health Centers is seeking a Care Coordinator to be part of the Clinical Care Team. The role focuses on coordinating care for patients with chronic conditions and behavioral health needs at the point of care, participating in daily huddles, and providing brief interventions to improve outcomes.

The position emphasizes patient-centered care, linkage to services, and collaboration within a diverse, community-based setting with a commitment to equity and quality improvement.

Qualifications

• Medical Assistant Training completed; one or more years of experience preferred.
• CPR certification within 90 days of hire.
• Valid WA State Driver’s License; access to reliable transportation.
• NCQA Certification is a plus for the role.
• Bilingual ability in Spanish is strongly preferred.

Responsibilities

• Coordinate care for patients with chronic conditions and behavioral health needs.
• Participate in daily huddles and identify patient needs per protocols.
• Provide brief interventions at point of care and link patients to services.
• Track and follow up on patient care plans and transitions.
• Maintain patient documentation in the EHR and communicate with providers.
• Assist with 340b medication and indigent patient programs as needed.
• Support group visits and care coordination across the care team.
