Job summary

Qualifications

• Must be able to work independently, prioritize workload and meet deadlines.
• Strong patient engagement and communication skills are essential.
• Bilingual in Spanish strongly preferred.
• Knowledge of medical terminology and behavioral health topics.
• Experience in ambulatory care is preferred.

Responsibilities

• Participate in morning huddles to anticipate patient needs.
• Identify gaps in care and support process improvements.
• Provide brief interventions at point of care for chronic conditions.
• Link patients to behavioral health and Sea Mar resources.
• Coordinate care transitions from ED or hospital settings.
