MEDICAL PROVIDERS PARTNERS PLLC in Lansdowne, PA is seeking a Chronic Care Coordinator to support our CCM program. You will engage with patients, physicians, and care teams to ensure ongoing, coordinated care for individuals with chronic conditions.

The role includes patient outreach, enrollment, documentation in the EMR, and timely follow-ups aligned with monthly CCM activities, while maintaining HIPAA compliance and care continuity.

Qualifications

• Requires valid driver’s license and ability to travel to assigned locations.
• Experience in healthcare, medical-office operations, patient coordination, or related field.
• Excellent communication and interpersonal skills.
• Strong organizational and time-management skills.
• Ability to work with an EMR and manage multiple patients and tasks.
• Strong attention to detail and documentation.
• Ability to work independently and as part of a healthcare team.

Responsabilités

• Conduct monthly outreach to patients enrolled in the Chronic Care Management program.
• Identify and assist with enrollment of eligible patients into CCM.
• Educate patients about the CCM program and obtain appropriate consent.
• Maintain and update individualized patient care plans.
• Coordinate care between patients, physicians, nurse practitioners, specialists, pharmacies, hospitals, home-health agencies, nursing facilities, and other healthcare providers.
• Assist with scheduling appointments, referrals, testing, and medication-related concerns.
• Follow up on hospitalizations, ER visits, rehabilitation stays, and transitions of care.
