BlueCross BlueShield of Tennessee, Inc. invites you to join its CHOICES Care Coordination Leadership Team as a Supervisor in the Middle Tennessee region.

Lead, mentor, and support a dispersed team of Care Coordinators serving Medicaid LTSS members, ensuring safe, home-based or facility-based care. This role requires strong clinical and operational guidance, regulatory compliance, and collaboration with providers and community partners.

Qualifications

• Experience 3 years - Supervisory experience required.
• 2 years - Experience in health insurance/managed care field required.
• Ability to conduct root cause analysis, plan, organize and coordinate multiple projects.
• Proficient in Microsoft Office (Outlook, Word, Excel and Powerpoint).
• Excellent oral and written communication skills.
• Strong interpersonal and organizational skills.
• LicenseRN with TN license or MSW license as described.

Responsibilities

• Monitor performance of staff including service performance and adherence to established utilization benchmarks.
• Provide and/or coordinate staff training for maximum performance and provide developmental opportunities.
• Supervise, direct, and evaluate a diverse group of health care professionals to assure effectiveness of consumer advocate, ancillary, discharge, institutional, care coordinator assistant and home and community-based care coordinator activities.
• May be required periodically to carry case load of members, when dictated by census fluctuations or staffing coverage.
• Responsible for communicating and mentoring members of the care coordination team to ensure long-term and home and community based care guidelines, policies, and procedures are followed.
• Valid driver's license.
• 90% day travel required.
• Employee may be required to participate in Runzhimer Program (auto reimbursement plan).
