Job summary

BlueCross BlueShield of Tennessee is seeking a remote Member Support Coordinator for TennCare CHOICES, focusing on long-term services and supports. You will screen, educate, and guide members and authorized representatives through eligibility and referral processes, working with internal teams and external partners.

The position requires a Masters in Social Work or RN/LPN licensure, 3 years of clinical experience, and strong communication and organizational skills.

Qualifications

• MSW (LCSW/LMSW/LAPSW) or RN or LPN required.
• Minimum 3 years of clinical experience.

Responsibilities

• Serve as interface for internal departments and external partners for enrollments, admissions and transitions.
• Review and assess reports (e.g., 834, ERC) and disseminate results to appropriate teams.
• Manage LTSS requests including referrals, screenings, records, enrollment, transitions and discharge planning.
• Submit completed requests to TennCare PAE and LTSS operations and track dispositions.
