Job summary
La Frontera Arizona, via EMPACT-SPC in Apache Junction, AZ, is hiring multiple Children’s Case Managers to provide crisis intervention, support, transportation and education to families and children. The role requires coordinating with partners to ensure continuity of care and referrals to community services.
The position is full-time with scheduling flexibility to meet client and family needs. BHT requirements with a degree or equivalent experience are required, along with a valid AZ driver
Qualifications
- Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
- Must have experience with the Child and Family Team process.
- Must have Fingerprint clearance card or be eligible to obtain one.
- Must have a valid AZ driver's license with no major infractions in the past three years.
- Bi-lingual (Spanish) is preferred.
- Bi-lingual Diff and Rural Diff per hour available.
- Must be at least 21 years of age at time of employment.
Responsibilities
- Provide direct service crisis intervention, support, transportation and education to families and children.
- Complete family needs assessments and refer to partners and community services.
- Coordinate communications with other departments or collaborators to ensure continuity of care.
