Job summary

EMPACT-SPC, part of La Frontera Arizona, invites applications for multiple Children's Case Manager roles in Apache Junction. The full-time position requires flexibility in scheduling to meet client and family needs.

The role provides crisis intervention, transportation, and education to families and children, coordinating with partners to ensure continuity of care and appropriate referrals.

Qualifications

• Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Experience with the Child and Family Team process.
• FingerPrint clearance card or eligibility to obtain one.
• Valid AZ driver’s license with no major infractions in the past three years.

Responsibilities

• Provide direct service crisis intervention, support, transportation and education to families and children.
• Complete family needs assessments and make referrals to partners and community services.
• Coordinate communications with agency departments or collaborators to ensure continuity of care.
