Job summary

EMPACT-SPC, part of La Frontera Arizona, is seeking dedicated Children's Case Managers to join our Apache Junction team. This full-time role delivers direct crisis intervention, family support, transportation and education to children and families, coordinating with partners as needed.

Requirements include a Bachelor's or higher with behavioral health experience (or A.A. with more experience, or a high school diploma with extensive experience), fingerprint clearance, AZ driver's license, and

Qualifications

• Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Must have Fingerprint clearance card or be eligible to obtain one.
• Must have a valid AZ driver’s license with no major infractions in the past three years.
• Bi-lingual (Spanish) is preferred.

Responsibilities

• Provide direct service crisis intervention, support, transportation and education to families and children.
• Complete family needs assessments and refer to collaborating partners and community services.
• Coordinate communications with other departments or partners to ensure continuity of care.
