Jefferson County Human Services is seeking Long Term Care Case Managers responsible for the client‑facing work of this division. Depending on the focus of their team, Long Term Care Case Managers provide a variety comprehensive case management which involves providing individualized support and coordination of services for clients, often with complex needs. This includes conducting assessments to determine eligibility, creating tailored care plans, linking clients to appropriate resources, and maintaining ongoing communication to monitor progress and adjust plans as needed. It requires a thorough understanding of community resources, policies, and regulations, as well as the ability to advocate effectively for clients. Requires ability to meet individuals at their home and in the community to complete assessment and monitoring activities. As a Children’s Case Manager, you’ll serve as a vital advocate and guide for families—helping them access services, understand their options, and create a coordinated care plan tailored to each child’s unique needs.
Schedule: 4 day/10‑hour work week (Monday‑Thursday). Hybrid, remote & in‑office as needed. Must live within a reasonable commuting distance from the Denver Metro area. Scheduled weekly hours: 40.
Target Hiring Range: $56,869 - $60,825.45 USD. Compensation will be determined based on education, experience, and skills.
