Job summary

Jefferson County Human Services seeks a Long-Term Care Case Manager to handle client-facing case management for the elderly, disabled, and children with chronic or terminal illness. You will evaluate eligibility for Medicaid plans, negotiate service options, and coordinate with providers to ensure appropriate care.

The role covers various programs including IDD, Non-IDD, and SGF Intake, with a hybrid schedule and opportunities to impact benefits enrollment and care planning.

Responsibilities

• Determine client eligibility by gathering collateral information from hospital, nursing home, long-term skilled home health and home-based services.
• Complete a face-to-face assessment of clients.
• Attend court appeal hearings to defend denied applications.
• Prepare all legal documents for the appeal hearing.
• Work with various Medicaid programs and negotiate rates with Medicaid providers.
• Maintain client care plans by reassessing for cost-efficient service options.
• Adhere to state cost containment regulations.
• Ensure family members are informed of options and make referrals as necessary.
• Complete billing paperwork.
