Job summary

Jefferson County Human Services is seeking a Long-Term Care Case Manager responsible for client-facing work in this division. The role involves assessing eligibility for Medicaid service plans, negotiating with providers, and supporting families of elderly and disabled clients across multiple programs.

The position is hybrid, with a 4-day/10-hour or 5-day/8-hour workweek depending on the team, and offers a comprehensive benefits package including health, PTO, retirement matching, and tuition

Qualifications

• Bachelor's degree in a related field.
• One year of work-related experience.

Responsibilities

• Determine client eligibility by gathering collateral information from hospitals, nursing homes, and home health providers.
• Complete a face-to-face assessment of clients.
• Attend court appeal hearings to defend denied applications.
• Prepare all legal documents for the appeal hearing.
• Work with Medicaid programs (HCBS, Elderly, Blind and Disabled, etc.).
• Maintain client care plans and reassess for cost-effective service options.
• Negotiate rates and plans with Medicaid providers.
• Complete billing paperwork.
