Job summary

MedStar Health is seeking a Health Services Coordinator to coordinate and facilitate access to health-related services for clients by utilizing available resources across our network in Maryland.

The role involves attending meetings, promoting health education, and supporting grant activities. A Bachelor's degree is required and 2+ years in healthcare with strong communication and cultural competency is expected.

Qualifications

• Bachelor's degree is required.
• Master's degree in Public Health or related field preferred.
• At least 2 years of healthcare experience.
• Strong cultural competency with target population.
• Effective verbal and written communication skills.
• Proficient in Microsoft Office, electronic health records, and patient scheduling systems.

Responsibilities

• Attend scheduled meetings and trainings related to safety, infection control, OSHA, CPR, etc.
• Assist staff in health education and quality improvement projects.
• Support department goals while adhering to policies, procedures, quality standards, and safety regulations.
• Develop and maintain relationships with external referral sources and internal contacts.
• Identify potential clients, perform initial needs assessments, and guide to health services.
• Maintain accurate client records in EMR and databases.
• Participate in orientation for students, residents, physicians, nurses, and other staff.
• Provide informal counseling and social support; assist with appointments and program applications.
• Reconcile care gaps with health plans to correct discrepancies.
• Screen target populations for eligibility.
• Support self-management goals and encourage goal setting.
• Collaborate with program team to ensure grants stay on track.
• Participate in quality and service improvement teams and community outreach.
