MedStar Health in Maryland seeks a program coordinator to coordinate access to health-related services for clients, promote health education and quality improvement, and ensure compliance with policies and regulations.

The role involves assisting staff, maintaining client records in EMR, supporting grant activities, coordinating with external referral sources, and providing counseling and navigation to services for diverse populations.

Competenze

• Bachelor's degree required.
• Master's degree in Public Health or related field preferred.
• 2+ years working in a healthcare environment.
• Strong understanding of cultural competency with target population required.
• Requires effective verbal and written communication skills.
• Must possess computer skills including proficiency in the use of Microsoft Office database systems electronic health records and patient scheduling systems.

• Attends all scheduled meetings including staff meetings and training classes for safety infection control OSHA CPR and others as required.
• Assists staff in promoting health education and quality improvement projects.
• Contributes to department goals and adheres to policies, procedures, quality and safety standards.
• Develops and maintains relationships with external and internal contacts.
• Identifies potential clients, completes initial needs assessment and follows up for guidance to health services.
• Maintains client records by documenting contacts, services and outcomes in EMR and registries.
• Supports orientation and education programs for students, residents and professionals.
• Provides informal counseling, education and social support to clients and assists with medical appointments and referrals.
• Reconciles care gaps for target populations; communicates with external stakeholders to correct discrepancies.
• Screens target populations for eligibility and supports self-management goals.
