Qualifications

• Must qualify as a Mental Health Practitioner in the State of MN.
• Valid driver’s license and proof of auto insurance.

Responsibilities

• Assess client and family needs by collecting, evaluating, and documenting information.
• Develop service plans in cooperation with family and other providers.
• Coordinate and make referrals for needed services.
