Job summary

Jewish Family & Children’s Service of Arizona is seeking a compassionate Case Manager to provide behavioral health case-management services to children across the Valley. The role includes guiding families, coordinating with DCS, schools, and other providers, and facilitating CFT meetings.

You will work with the clinical team to ensure that service plans and related documentation are completed accurately and on time, with weekly client contact, home visits, and discharge planning as core duties.

Qualifications

• Requires compassionate, patient and adaptable case manager.
• Strong communication and collaboration with families and partners.
• Experience coordinating with schools, DCS, probation/parole and other agencies.
• Ability to document and manage paperwork accurately.

Responsibilities

• Provide weekly contact with clients including home/community visits.
• Assist youth with discharge planning and service navigation to improve stability.
• Coordinate services and collaborate with partner agencies and schools.
• Lead or facilitate Child & Family Team (CFT) meetings.
• Ensure Integrated Service Plans and notes are completed accurately and timely.
