Jewish Family and Children’s Service (JFCS) is seeking a High Needs Case Manager in Phoenix, AZ. The role focuses on youth receiving behavioral-health services, guiding families, and coordinating care to support reunification and stability.

Responsibilities include weekly client contact, home visits, discharge planning, and collaboration with DCS and schools. Candidates should meet local education/experience requirements and hold necessary clearances and a valid AZ driver’s license.

Qualifications

• Meets one of the education/experience requirements.
• Behavioral health experience required or equivalent.
• Driver license and reliable transportation required.

Responsabilités

• Provide case management services to children in behavioral-health system.
• Maintain weekly contact with clients and perform home/community visits.
• Assist youth in discharge planning and accessing intensive services as clinically indicated.
• Coordinate with agencies (DCS, Probation/Parole, DDD, schools, providers).
• Facilitate Child & Family Team (CFT) meetings and required paperwork.
