Lighthouse Behavioral Wellness Centers is seeking a dedicated Care Coordinator to empower families navigating mental illness, trauma or addiction. Under supervision, you will organize the Child and Family Team, coordinate services and supports, and help families create strength‑based, culturally informed care plans.

The role requires travel to schools and communities, home visits when needed, and flexible hours to meet family needs. Strong collaboration and communication are essential.

Lighthouse Behavioral Wellness Centers is just such a place, guided by its core values of hope, dedication and community.As a Care Coordinator you will empower, uplift and help others heal from the many forms of mental illness, trauma or addiction. Those who have recently experienced crisis and/or hospitalization, may need additional guidance as they maneuver multiple systems, engagement with new treatment team members and adjust to changes outside of the clinical setting.To meet the increasing needs of our communities it is imperative that all team members understand and successfully reach and maintain the expected performance standard levels. In order to achieve total and sustainable success each person must to do their part. Lighthouse will do its part in providing any and all necessary tools and training required to help team members be successful.DEFINITION:Under immediate to general supervision, provides targeted support services, including case management, rehabilitation and wellness services, a system of care that is child centered with the needs of the child and family dictating the types and mix of services provided and to assist in seamless engagement into outpatient services and aftercare to prevent further out-of-home or out-of-school placement. To be effective, the Care Coordinator may work with the identified consumer and family in their homes, schools and communities, incorporating the family and other social supports in treatment and be focused on the strengths as well as the consumers preferences and culture. This position is supervised by the respective Director of Outpatient Services.As a WRAP Care Coordinator, you are more than someone who connects families to services—you are the person who helps bring the right people together, keeps the team moving forward, and helps families build a plan that reflects their strengths, needs, culture, and goals. WRAP is built around the belief that families should not have to navigate complex systems alone, and the Care Coordinator plays a critical role in organizing the Child and Family Team, coordinating services and natural supports, and helping everyone stay focused on what matters most to the family. Lighthouse is looking for someone who is energetic, proactive, organized, and willing to take initiative. This role requires Dedication—including flexibility to work outside normal business hours when needed, travel to schools and community settings, conduct home visits, and meet families where they are. We are looking for a self-starter who can build relationships, overcome barriers, bring people together, and help families develop the confidence and support system they need to succeed long after formal services end.EDUCATION AND EXPERIENCE:A Bachelor or Master Degree in Education, Psychology, Social Work, Sociology, Criminal Justice or similar behavioral health field approved by ODMHSAS; orA current license as a registered nurse in Oklahoma with behavioral health experience; orA Bachelor’s or Master’s degree in any field with 6 months of behavioral health experience; or60 college credit hours in any field and 12 months of behavioral health experience; orA high school diploma or equivalent and a total of 36 months of direct, documented experience working with persons who live with mental illness and/or substance use issues.Applicants must be able to provide documented proof from previous employers to justify behavioral health experience.One year of work experience with juveniles in the mental health or social service field preferred.Behavioral Health Case Management Certification and Behavioral Health Wellness Coach Certification is required within the first three months of employment and must be kept current annually.Current certification as a Behavioral Health Case Manager Il as described in ODMHSAS Standards ad Criteria for Certified Behavioral Health Case Managers 450:50-3-1; andCurrent certification as a Behavioral Health Wellness Coach as described in ODMHSAS Standards ad Criteria for Certified Behavioral Health Case Managers 450:50-5-145.An expectation of core competencies in relation to children and families, as well as individuals with co-occurring disorders, trauma informed care and collaborative care.KNOWLEDGE AND SKILLS:Knowledge of the principles and practices of social work and/or psychology; and of current social and economic problems, principles of family economics and budgeting, human relations, community, state and national health and welfare resources, social assistance requirements, and psychological terminology.Knowledge of transitional and/or aftercare needed after a crisis or hospitalization and the unique situations that may arise as consumers attempt to engage in outpatient services.Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organize and present facts effectively.Knowledge of the elements of personal wellness and how to collaborate with others to identify their personal strengths and goals within the Eight Dimensions of Wellness.Able to establish working relationships within an integrated team to coordinate and facilitate care by assessing consumer needs, evaluating treatment options, creating care plans, coordinating care with internal and external providers, and monitoring progress.Ability to work effectively and courteously with children and families from various cultures and backgrounds, and to exercise good clinical judgment.Specialized training and/or certification in mental health services or related field as may be required in order to provide adjunctive services, ie. Systems of Care, Wraparound trainingJOB DUTIES AND RESPONSIBILITIES:This position is responsible and held accountable for the following duties:Provide varying types of individualized case management, rehabilitative and wellness services in various settings to individuals, families and groups to promote emotional or psychological change to alleviate mental disorders and facilitate recovery and encourages engagement in the treatment setting.Schedule, coordinate and facilitate wraparound family team meetings and ensure that formal and informal supports are included and part of the team.Complete Strengths Needs Cultural Diversity (SNCD) Assessments, Care Plans, Functional Assessments, Crisis Plans and Safety Plans.Completes OKSOC Assessments and other specified documentation as assigned.Enters data into the YIS system according to ODMHSAS requirements and timelines.Provide crisis management and stabilization as assigned.Monitors and evaluates the needs and abilities of the consumer and assists them to function more adequately through use of their own strengths and through use of community resources.
