Job summary

Lighthouse Behavioral Wellness Centers in Durant, OK seeks a Care Coordinator to provide targeted case management, rehabilitative and wellness services for children and families. Qualifications

• Education or experience in behavioral health as required by ODMHSAS.
• Certification in Behavioral Health Case Management and Wellness Coaching within 3 months of employment.

Responsibilities

• Provide individualized case management, rehabilitation and wellness services in various settings.
• Schedule and facilitate wraparound family team meetings with formal and informal supports.
• Complete SNCD Assessments, Care Plans, Functional Assessments, Crisis Plans and Safety Plans.
• Enter data into the YIS system according to ODMHSAS requirements and timelines.
• Provide crisis management and stabilization as assigned.
• Develop referrals to community resources for housing, employment, medical and wellness services.
• Collaborate with families and professionals to coordinate care and monitor progress.
