Bellefaire JCB in Shaker Heights, OH, seeks a Child, Adolescent, and Family Therapist to provide Intensive Home Based Treatment. The role requires licensure, a Master’s in social work, and experience with youth and families. Work occurs in home and community settings with some evenings.

Training and strong documentation practices are expected. Responsibilities include assessments, treatment planning, crisis coverage, and coordinating services across stakeholders to improve client outcomes.

Qualifications

• Master’s degree in social work or related field required.
• Valid Ohio LSW, LMFT or LPC license required.
• Experience working with children, adolescents and families; chemical dependency treatment.

Responsibilities

• Accept assignment of cases to provide Intensive Home Based Treatment for clients, families, and with significant others as needed.
• Provide Psychiatric Diagnostic Assessment and Intensive Home Based Treatment including evaluations, treatment planning, crisis work and 24/7 crisis coverage.
• Support and educate families regarding clients’ mental illness and coordinate services identified on the client’s Treatment Service Plan (ITP).
• Conduct individual interventions; monitor symptoms and enhance social support networks.
• Coordinate evaluations and services; ensure follow-up to meet client needs.
• Attend flexible workweeks with on-call coverage evenings/weekends on a rotating basis.
• Maintain required trainings, certifications and licensure per regulations.
• Maintain clinical records with timely documentation.
