Community Bridges, Inc. (CBI) is seeking a Child/Adolescent Integrated Case Manager to support youth and families within Arizona’s behavioral health programs. The role focuses on continuous engagement, needs identification, and coordination with the treatment team to address barriers to care.

Responsibilities include ensuring follow‑up services before discharge and applying recovery‑oriented approaches for both children and adults as needed. This position is based in Springerville/St.

Qualifications

• High School Diploma or GED required; Associate’s Degree preferred.
• 1–3 years of health care related work experience preferred but not required.
• 1–3 years of substance abuse/mental health recovery‑oriented service preferred but not required.
• Minimum of six months recovery from substance use and/or mental health disorders preferred.
• Experience working with children/adolescents preferred.
• Behavioral Health Technician (BHT) certification required.
• Arizona Fingerprint Clearance card required.
• Current AZ Driver’s License (valid and in good standing).
• Clear 39‑month Motor Vehicle Record (MVR).

Responsabilités

• Provide ongoing support and engagement for child/adolescent within CBI services.
• Coordinate with treatment team and community/external agencies to address client needs.
• Ensure supportive and follow‑up services are provided prior to discharge from CBI programs.
• Address client specific behavioral health needs, objectives, and goals with adults when applicable.
