Job summary

Cantex Continuing Care Network in San Antonio, TX is seeking an LVN/LPN Charge Nurse for weekend shifts. You will oversee patient care within the LVN/LPN scope, observe patient conditions, document changes, administer medications and treatments, and supervise CNAs to ensure compassionate, safe, and consistent care for residents.

Graduation from an accredited nursing program and a valid Texas LVN/LPN license are required.

Qualifications

• Must be a graduate of an accredited nursing program and hold a valid LVN/LPN license.
• Texas LVN/LPN license in good standing is required.

Responsibilities

• Oversee and participate in patient care within the LVN/LPN scope.
• Observe, assess, and report patient conditions; document per policies.
• EMR documentation of patient assessments and changes.
• Administer medications and treatments as prescribed.
• Supervise CNA staff to ensure quality care.

Qualifications: Responsibilities:

• As an LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse, you will participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observes, assesses, and reports Patient conditions/changes and documents in accordance with Cantex’s Clinical Policies & Procedures and state/federal guidelines & regulations.
• EMR documentation relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices.
• Ensures implementation of care plans as directed and administers medications and treatments as prescribed.
• Participates in assigning duties and supervises the delivery of appropriate clinical care by the CNA staff to ensure highest practical well-being is maintained by patient.

Requirements:

• Graduation from an accredited Nursing School
• Valid LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse License
