Job summary

Cantex is seeking a dedicated LVN/LPN to join our Coronado at Stone Oak team in San Antonio, TX. You will participate in and oversee patient care within the LVN/LPN licensure scope, observe patient conditions, and document assessments in the EMR.

You will administer medications, implement care plans, and supervise CNAs to ensure the highest level of patient well-being in a supportive facility environment.

Qualifications

• Graduation from an accredited Nursing School.
• Valid LVN/LPN license within state.
• Experience in clinical nursing care preferred.

Responsibilities

• Observe, assess, and report patient conditions and changes.
• Document patient assessments and care in EMR per policies and regulations.
• Administer medications and treatments as prescribed.
• Supervise CNA staff and ensure delivery of appropriate clinical care.
• Oversee care plan implementation within licensure scope.

Qualifications and Responsibilities

• As an LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse, you will participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observes, assesses, and reports Patient conditions/changes and documents in accordance with Cantex’s Clinical Policies & Procedures and state/federal guidelines & regulations.
• EMR documentation relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices.
• Ensures implementation of care plans as directed and administers medications and treatments as prescribed.
• Participates in assigning duties and supervises the delivery of appropriate clinical care by the CNA staff to ensure highest practical well-being is maintained by patient.
