Job summary

Cantex Continuing Care Network in San Antonio, TX is seeking a licensed LVN/LPN to join our Skilled Nursing team on a PRN basis. You will participate in and oversee patient care within the scope of your licensure, delivering compassionate, high-quality nursing services.

This role emphasizes accurate EMR documentation, timely observation of patient conditions, and collaboration with CNAs to maintain optimal well-being.

Qualifications

• Graduation from an accredited Nursing School.
• Valid LVN/LPN license in Texas.
• Ability to oversee patient care within scope of licensure.

Responsibilities

• As an LVN/LPN, participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observes, assesses, and reports patient conditions/changes and documents per policies and regulations.
• EMR documentation related to patient assessment and change of condition.
• Ensures implementation of care plans and administers medications as prescribed.
• Participates in assigning duties and supervises CNA staff to ensure patient well-being.

QualificationsResponsibilities:

• Ensures implementation of care plans as directed and administers medications and treatments as prescribed.
• Participates in assigning duties and supervises the delivery of appropriate clinical care by the CNA staff to ensure highest practical well-being is maintained by patient.
