Job summary

Cantex Continuing Care Network’s Skilled Nursing facility in San Antonio, TX is seeking a dedicated LVN/LPN to join our team on a PRN basis. You will participate in and oversee patient care within the LVN/LPN scope, ensure accurate assessments, and document changes in the EMR in line with policies and regulations.

You will administer medications and treatments as prescribed, help supervise CNAs, and support care plans to promote patient well‑being.

Qualifications

• Graduation from an accredited nursing program.
• Valid LVN/LPN license in good standing.
• Experience in long-term care settings preferred.

Responsibilities

• Participate in and oversee patient care within the LVN/LPN licensure scope.
• Observe, assess and report patient conditions; document changes per policies and regulations.
• EMR documentation related to patient assessment and changes in condition; document care delivery.

QualificationsResponsibilities:

• As an LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse, you will participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observes, assesses, and reports Patient conditions/changes and documents in accordance with Cantex’s Clinical Policies & Procedures and state/federal guidelines & regulations.
• EMR documentation relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices.
• Ensures implementation of care plans as directed and administers medications and treatments as prescribed.
• Participates in assigning duties and supervises the delivery of appropriate clinical care by the CNA staff to ensure highest practical well-being is maintained by patient.

Requirements:

• Graduation from an accredited Nursing School
• Valid LVN/LPN - Licensed Vocational Nurse or Licensed Practical Nurse License
